Act 32 requires all employers to submit the following information on all filings: Employee's full social security number, full name, full address, gross wages, and local Earned Income Tax (EIT) withheld for the quarter, work site PSD and the resident PSD code.
Certificate of Residence (To be used by employers when a new employee is hired or when a current employee notifies the employer of a name and/or address change)
